Schedule: Mon-Fri 8am- 4pm
Under the direct supervision of the Healthcare Supervisor/RN, provide medical services, training, and counseling to adults with intellectual/developmental disabilities.
Perform skilled nursing, health education, and other pertinent nursing services to people receiving services.
Assist and attend to personal care needs of the individuals served as needed.
Administer medications and treatments following Medication Administration Program (MAP) guidelines.
Follow medical protocols, treatment plans, medication treatment plans and communicate health changes in a timely manner.
Assist in maintaining up to date medical information for each member’s case file including but not limited to: routine vital signs; current physical exams; nursing interventions and progress notes; doctor’s visit notes; hospitalization notes and discharge summaries.
Serve as an advocate for the people served.
Perform other work related duties as assigned.
Possess a valid Massachusetts Licensed Practical Nurse (LPN) License.
One year of nursing experience.
Working knowledge of computer skills, MS Office.
Previous experience working with individuals with Intellectual/Developmentally disabilities and/or Psychiatric conditions preferred.
Requires a valid driver’s license and acceptable driving record.
Current CPR/First Aid Certifications required or be able to obtain these within thirty (30) days of hire.
Pass criminal background check (CORI) and National Fingerprint Background Check.
