This position provides community-based direct services to individuals enrolled with our Collaborative Care Coordination program. Consumers enrolled in this program are working towards a variety of goals based on their needs, including but not limited to: housing, employment, education, linkage to benefits, legal and social supports as well as behavioral and physical health providers.
Responsibilities may include assisting individuals to access medical, and behavioral health services, assisting individuals in acquiring financial and legal benefits, linking individuals to appropriate community based services and providing transportation to appointments or activities as related to the consumer's goals. Mental Health Counselors participate in team meetings and communicate regularly with other providers involved in the care of the assigned individuals. This position is eligible for a wide range of employee benefits.
Excellent communication, interpersonal and organizational skills required
Basic computer fluency including Microsoft Office required
Valid driver's license and reliable personal vehicle required for local travel
Bilingual experience encouraged to apply

Bachelor's Degree in mental health related discipline required
