We have an opening on our Hospice team for a Clinical Support/Triage LPN. This position supports hospice operations and is responsible for managing all aspects of hospice patient calls ensuring that our customers are treated with the highest level of customer service.
Facilitate the collection of information required in triaging hospice patient inquiries
Provides support to the Clinical Manager and to the clinical staff in the field by coordinating phone calls to physicians, patients, and families regarding medication, treatments and management of symptoms
Works closely with Hospice Liaison Team in the coordination of referrals
Ensures that all referrals being processed for admission comply with the Medicare Conditions of Participation
Demonstrates exceptional customer service to patients, families, referral sources, co-workers and other customers
Assures compliance with all federal and state laws as well as agency compliance policy in the prevention of fraud and abuse
Skills and Abilities:
Please have exceptional customer service skills. You will work with a wide variety of stakeholders
Please have exceptional communication skills. You will be managing the fast-paced flow of information to and from all stakeholders via phone, email, text and fax. Must be able to communicate effectively and courteously
Please be a problem solver. Must be able to troubleshoot quickly and on the fly
Must be an expert at multi-tasking in a fast paced environment with constantly changing priorities
Education and Experience:
LPN license required
Please have 2+ years in an office setting. Hospice experience preferred
Please be fluent in Microsoft Office
