Fidelity House Human Services is a nonprofit organization committed to supporting people with developmental disabilities and their families. We are proud to offer 20 community residences throughout Lawrence, Methuen, Haverhill, Georgetown and Andover along with specialized services across 68 cities and towns in Northeastern Massachusetts.

Caring person needed to provide assistance to families of individuals within our Children and Family Services Division as they go about their daily lives - whether that is navigating the community, support services, attending family meetings or within Massachusetts' State Agencies. The ultimate goal: individuals who are able to achieve the richest and fullest life possible.
SCHEDULE FOR THIS POSITION: 2 Days (15 Hours Per Week)
◾The Service Navigator is responsible for overseeing designated support program(s). Supports offered should be in response to family directives and offer meaningful supports that help the family and individual in their daily lives.
◾Identify and develop resources in the community where individuals are welcomed, can experience varied, normalizing opportunities to learn and grow from, and that encourages integration in the broader community.
◾Act as a liaison between families/individuals and others as requested by the family.
◾Attend family meetings, as indicated.
◾Support the Family Advisory Council and advise, as requested.
◾Provide supervision to direct care staff, assume responsibility for the delivery of quality services and ensure delivery of services occur as designed through the program model.
◾Maintain regular contact with direct care staff to ensure open communication and ensure adequate staffing to meet family and individual needs. Report concerns, as necessary, to supervisor.
◾Identify and promote training opportunities for direct care staff.
◾Oversee contract utilization and other associated program costs.
◾Participate in the development and implementation of program initiatives.
◾Complete intake and referral procedure, as necessary.
◾Submit all required documentation on a monthly basis to supervisor and/or DDS.
◾Other duties as indicated and assigned by supervisor.
◾Meet with supervisor, as necessary.
◾Complete other tasks, as needed or assigned.
◾3 years' experience working with individuals with disabilities
◾Bachelor's Degree in Special Education, Psychology, Social Work, Human Services or related
◾Bilingual in Spanish and English, required
