Position Title: Community Coordinator
Hours: Monday-Friday 8AM-4:30PM

Triangle has an immediate opening for a Community Coordinator in our Downtown Malden location. The Community Coordinator is responsible for developing and implementing employment, independence, and life skills experiences and activities for adults with disabilities in the community. This is a full-time, entry level, direct care position.
Essential Duties and Responsibilities:
Supervises groups of participants in a variety of settings including work sites, volunteer sites, recreational outings, and other community outings, typically between the hours of 9am-3pm
Participates in the planning and implementation of vocational exercises to foster personal and professional growth
Facilitates in-house and community activities, keeping participants active and engaged
Provides job coaching as needed at employment or volunteer sites (employment sites can on occasion include: days, nights, weekends and holidays)
Transports individuals to and from community activities/employment sites
Educate participants during teachable moments in areas of socialization, community membership, daily living, etc.
Develops and maintains diverse volunteer, vocational, and educational opportunities for individuals
Develops and implements independent living, employment, self-advocacy, and soft skills into daily activities and curriculum
Prepares or assists in the preparation of required assessments and reports (i.e., incident reports, safety assessments, progress summaries, etc.)
Intervenes in any unsafe interactions or behavioral incidents that occur to ensure individuals safety
Participates in ISP meetings with Department of Developmental Services, participants, and their families or other members of their support team as needed
Takes direction from Case Manager and Program Monitor to collect accurate data as needed for ISP's, behavior plans, monthly progress notes, etc.
Tracks and submits billable activities utilizing Penelope software as needed
Collaborates with other Community Coordinators and Program Monitor to ensure quality service delivery
Reports progress and issues to Program Monitor and Case Manager on a regular basis
Know emergency procedures and basic emergency facts regarding all participants
Participates in identified training and professional development opportunities, in addition to all required trainings
Positively represents Triangle to customers, family members, and community organizations
Participates in staff meetings with colleagues and community partners as needed
Meets with Program Monitor on a monthly basis for supervision
Operate 12-passenger van
Other duties as assigned
High school diploma or equivalent
Some college in related coursework and/or one to three years' experience in disability/human services
Massachusetts driver's license and clean driving record
Willingness to submit to State and Federal Background Check
First Aid/CPR certification (will provide this training)
