We currently have an opening for a full-time Patient Care Access Coordinator in the Salem Administrative Office. Reporting to the Director of Quality Improvement, the Patient Care Access Coordinator to address gaps in care. The PCAC will provide outreach to patients who are in need of routine care, routine screening, and/or chronic disease management, to ensure engagement in primary care. The PCAC will follow up with patients who no show for appointments and will identify and address the barriers that exist for patients in showing up for appointments. The PCAC will also serve as a liaison between the Population Health Manager and the patient care teams, and will provide data entry support to ensure accurate documentation of activities performed.

•Provide consistent, reliable and excellent customer service.
•Communicate effectively, efficiently, and professionally over the phone with patients.
•Schedule appointments via Centricity and Dentrix scheduling systems.
•Verify and update demographic and insurance data in Electronic Medical Record.
•Take accurate and detailed messages for other health center staff and deliver promptly.
•Make calls to patients about upcoming appointments and/or to reschedule appointments.
•Make calls to patients who are in need of routine care, routine screening, or chronic disease management.
•Provide data entry support to the Population Health Manager and Primary Care Teams.
•Enter data around coding gaps and quality gaps via the Identifi Practice system.
•Contribute to the team effort by supporting all staff members and maintaining an open and positive attitude.
•Assist with patient care wrap-around services, such as referral coordination and arrangement of transportation (e.g. PT1 forms).
•Other duties deemed necessary or appropriate by supervisor in order to fulfill NSCH's mission.
•Attend departmental meetings, including Quality Improvement committee.

◦High school graduate or equivalency
◦Minimum 2+ years of experience in an ambulatory health care setting, customer service, or related department preferred (i.e. call center)
◦Bi-lingual in Spanish/Portuguese and English strongly preferred
◦Demonstrated work history of reliability, good attendance and positive attitude
◦Ability to work independently as well as part of a team
◦Willingness and ability to learn new technologies Digium/Switchvox phone system, GE Centricity for medical appointments, Dentrix for dental appointments
◦Ability to communicate effectively with diverse staff and patients
◦Warm and professional telephone manner; good memory and patience
◦Ability to multi-task and follow-up to complete tasks
◦Consistently exhibits behavior and communication skills that demonstrate NSCH commitment to excellent customer service, including quality, care and concern with each and every internal and external customer
