Congenial Healthcare, a newly-formed physician organization comprised of 5 well-established practices on the North Shore, is seeking an RN to establish a Care Management program.
The RN Care Manager, as a member of the central health care team at Congenial, collaborates to proactively enhance the delivery of patient care services along the continuum of care. Utilizing a patient centered approach; the care manger directly interfaces with physicians, health care teams, patients and their care givers in managing their care. The RN High Risk Care Manager meets patient's needs efficiently and expeditiously by continuously working to improve the patient's experience, helping to ensure high quality patient care and reducing cost. Through broad knowledge of clinical care and systems management, the RN Care Manager evaluates, predicts, and facilitates the trajectory of patient care, monitors utilization of services, and identifies medical and social drivers of utilization. The RN Care Manager will work across the continuum helping patients and families to understand health care options, create and promote adherence to a care plan, and increase the patient's ability for self-management and shared decision making and connect patients to the appropriate community resources.
Required: The ideal candidate will be interested in supporting and influencing the development of a new Care Management program, with potential for growth as the program team expands over time. A desire to build and shape a burgeoning program from the ground up. Strong assessment and problem solving skills. Strong interpersonal skills. Ability to work independently with minimal supervision. Goal oriented and accountable. Demonstrated organizational skills. Demonstrated ability to work in a complex and continually-changing environment. Strong oral and written communication skills. Demonstrates appropriate communication skills for the patient population served. Good computer skills. Ability to quickly demonstrate competency in various software applications. Family Medicine experience preferred. Ability to work well with people of various ages, background, ethnicities, and life experiences. Possess a robust understanding and management of chronic health conditions and population management. Identifies risk prone or problematic situations and promptly seeks manager assistance, if needed.
Must possess reliable transportation for local travel and a valid driver's license. Completion of an accredited Nursing degree program. Minimum of 2 years acute care experience. 2 years of other experience: may include home care, post acute-care settings or care coordination/case management experience. Supervisory experience a plus.
