The Direct Support Professional and Support Specialist is responsible for completing the daily tasks in the residence and the community necessary for ensuring the individuals in the program receive the appropriate care as outlined by Individual Support Plans, medical orders for medication and treatment and agency policies and procedures. The DSP communicates regularly with other members of the team and managers to ensure the highest quality of care is being provided in the home. The DSP ensures individuals are provided with opportunities for choice, community participation, and skill development towards the goal of attaining the greatest independence possible.

Individual Services
 Provides opportunities for individual choice and independence
 Implements individual goals as outlined in Provider Support Strategies
 Supports individual to participate in all scheduled activities in and out of the home
 Prepares meals/assists individuals in preparing meals
 Completes laundry/assists individuals in completing laundry
 Completes household cleaning chores/assists individuals with completing cleaning tasks
 Advocates for the needs of individuals in the program
 Delivers medication/provides medical treatments as outlined in orders by medical providers (Med Cert
 Support individuals to exercise their Human Rights, reports any instances of abuse or neglect as mandated.
 Accompanies individuals on medical appointments (scheduled and emergency), communicating all relevant information to doctors, ensuring that all required paperwork is returned to the program
 Completes all medical treatments according to written orders (medications, treatments, diets, etc.)
 Communicates all important information (medical, behavioral, ISP) with supervisors
 Participates in fire drills (runs drills as assigned, follows all written procedures outlined in EESP, completes fire drill report).

Program Site
 Performs routine and non-routine cleaning chores to ensure programs are clean and safe.
 Identifies and reports any safety issues to appropriate supervisors (Site Manager/Cluster Manager/Residential Director

 Participates in regularly schedule team meetings
 Attends all assigned trainings (in person and online in the Cornerstone system)

 Accurately records all data related to ISP goals
 Records all necessary medical/medication information (i.e. med administration, medical progress notes, vital signs, etc.)
 Completes shift notes each shift
 Reads and responds to emails in RHD email system
 Completes all required reports (i.e. incident reports, accident reports, progress notes)
