Step by Step Supportive Services (SBS) is a not-for-profit, family-supported organization that empowers individuals to find their own paths to independence while dealing with the complications of life with mental illness. SBS is an intimate organization comprised of friends, family and passionate and caring professionals who value the importance of family involvement in the recovery process and go to great lengths to foster a safe, supportive environment. Our goal is to help each person achieve self-awareness and independence. We are seeking a clinical case manager to work Sunday through Thursday 12 pm-8 pm, with some flexibility around hours. It is incumbent upon the clinical case manager to provide our clients with the support necessary to reach these goals in a safe and caring environment while meeting the needs of our staffing levels.


Responsibilities

-Develop and review Individual Support Plans for assigned clients.
-Complete documentation of services provided by end of every shift.
-Provide intensive support and advocacy as needed.
-Participate in weekly staff meetings, weekly individual supervision, and additional trainings as needed.
-Coordinate and facilitate evening community meals and facilitate conversation and cleanup amongst guests.
-Provide transportation for clients as needed to medical or other appointments, classes, gym etc.
-Provide direct and indirect support to SBS clients in milieu including crisis management.
-Provide case management tasks which include but are not limited to:
-Financial management, creating and implementing an individualized budget for each assigned client
-Assist clients in developing structure through productive activity- clients should maintain 20 hours a week. ----
-Assist clients in finding appropriate volunteer or job opportunities
-Weekly meal planning and rotation of groceries
-Support clients with overall wellness goals.
-Assist clients in time management including maintaining their own schedule of appointments, classes, work, etc.
-General apartment organization and maintenance
-Advocate for client in obtaining and maintaining additional outside benefits
-Provide support with executive functioning skills
-Life skills coaching
-General daily support as needed
-Other services on an individualized basis
-Coordination of Services as needed with client's treatment team.
-Remain available and accessible during designated on-call rotation weeks.
-Plan, facilitate and lead psychoeducational and skill building groups on a quarterly basis.
-Create and maintain a safe and supportive atmosphere by following program policies, and procedures.
-Ensure effective communication of priority information to all necessary staff and shifts.
-Respond appropriately to emergencies including contacting appropriate staff and supervisor(s), interacting with police, fire, and medical personnel as needed.
-Transportation of clients to and from appointments and activities as needed.
-Respond to all emails and phone calls within 24 hours.
-Ensure the health, safety, and well-being for each client.
-Other tasks and assignments that support the mission of Step By Step.


Qualifications

-Master's Degree in relevant human services field
-Professional licensure or ability to apply for licensure within 90 days of hire, i.e. LCSW/ LICSW or LMHC
-At least one year of experience.
-Computer literate.
-Ability to consistently meet all necessary background and credentialing requirements.
-Valid driver's license. Must have reliable transportation to and from work.
