Description & Responsibilities:

The Registered Nurse provides services to members in accordance with the State Nurse Practice Act, Accepted professional standards of practice, Adult Foster Care service regulations, and Agency policy. This position provides nursing services, utilizes a comprehensive base of nursing theory and nursing process, and communicates/documents observations and assessments. Duties include, but are not limited to:
Determining if the Agency is able to meet all of the members' needs to ensure AFC services are properly being delivers;

Developing and reviewing the AFC Service Plan of Care;
Managing, in conjunction with the care manager, designated caseloads;
Reporting changes in the member’s condition;
Providing timely assistance and responding to urgent or emergency needs of the member;
Conducting on-site visits with members either monthly, every other month, or as needed;
Completing documentation pertaining to AFC services provided;
Participating in discharge and transition planning and implementation;
Initiating interdisciplinary referrals;
Selecting, supporting, and supervising caregivers in conjunction with the care manager;
Reviewing caregiver care logs;
Assisting and educating members and caregivers with all aspects of hygiene and health concerns;
Demonstrating teamwork, cooperation, effective communication, and confidentiality in all aspects of the job.

Qualifications:

Must be fully licensed by the Massachusetts Board of Registration in Nursing;
Must have at least two years of recent experience in the direct care of elders or adults with disabilities;
Bilingual skills preferred, especially in Vietnamese;
Cultural competency and sensitivity;
Strong communication and interpersonal skills;
Compassionate, flexible, and team player;
Knowledge of basic computer skills; and,
Satisfactory status on background check.
