RN and LPN Nurses: Hearthstone is in search of a full-time Wellness Coordinator to work in our memory care unit. Compensation starts $27.00 - $32.00 per hour, depending upon experience. A $1,000 signing bonus is being offered once training and orientation has been successfully completed.
In this full time position, you will be using your nursing skills in coordinating resident's health related needs in keeping with our philosophy of creating a residential atmosphere where people with cognitive impairment can flourish.
To qualify for this role, you will have 2+ years of nursing experience caring for those with dementia. You will also have least two or more years of oversight and clinical experience and a passion for providing quality healthcare to the geriatric/memory impaired population. As the Wellness Coordinator, you will be using your nursing skills to provide support and leadership to the residents, their loved ones, and staff. You will be involved in the supervision and training of Resident Companions, working with outside providers, vendors, and ensuring regulatory compliance.
Additional responsibilities include:
•Participating in the initial assessment of potential residents
•Following through on service plans and working collaboratively with all involved members of the team and families
•Conducting regular Resident Companion training, support and competency evaluations
•Working on a rotating schedule including weekends, holidays, and being on call
As our Wellness Coordinator, you are flexible, have strong assessment skills, and a passion caring for individuals with dementia in addition to the education and nursing experience in order to perform successfully in this position.
Additional requirements include:
•Able to work weekends, variable hours during the day to ensure overlap
•On call every other week
•Valid LPN or RN nursing license
•Able to assess residents with dementia, coach and instruct Resident Companions, interact with resident's family and educate them about the disease
•Knowledge of state Assisted Living regulations
