What will you be doing?
Hire, train and supervise case management team.
Ensure all levels of caregiver services are being provided in a timely, safe, compassionate and caring manner to clients, in accordance with all Always Here policies and procedures and MA board of nursing
Perform Client Assessments
Create client-specific Care Plans; shares findings and planned actions with client/client representatives, client providers and office staff in a collaborative manner.
Address client needs in a holistic manner referring internally and externally when needed.
Coordinate with the client representatives, care managers, caregivers, and office staff as necessary to provide continuity of care and the best care possible for clients.
Work with the Human Resources Manager to ensure that all caregivers are trained and possess the skill set and certifications to perform and administer the required care.
Routinely collaborate with Human Resources Manager regarding all caregiver disciplinary issues as well as to identify upcoming staffing requirements and ensure that Always Here is well-positioned to meet those needs.
Perform case manager reviews.
