United Cerebral Palsy of MetroBoston (UCP) is a progressive, non-profit agency committed to advancing the independence, productivity and full citizenship of people with disabilities. UCP is looking for an RN or LPN to fill the position of Healthcare Coordinator at its Day Habilitation Program.
Responsibilities: Full time position Healthcare Coordinator position located at a Day Habilitation program in Watertown. This position will oversee and administer medications, tube feedings, breathing treatments, wound care assessments, as well as monitor vitals and health of all individuals within the program. This position will provide comprehensive annual nursing assessments, quarterly and monthly medical reviews for all individuals, as well as maintain written correspondence with providers, families, etc. This Coordinator will work closely with an Interdisciplinary team illustrating teamwork and hands on training to all members of the day habilitation team, including on staff OT's, PT's, SLP's and a Behavior Therapist. This position is a liaison to all families, caregivers and outside providers regarding health related concerns, having direct contact with families, caregivers, physicians, and outside vendors. This position will work closely with another UCP Healthcare Coordinator/RN on coordination of nursing needs between both day program sites as needed, oversight of nursing documentation, development of medical policies and masshealth regulation oversight. Qualifications: Currently Massachusetts licensed with preferred experience in the area of developmental and physical disabilities.
