We are looking for upbeat, conscientious and hardworking Case Manager for our site in Stoneham!
Exciting full-time opportunity to obtain leadership and human services experience while making a difference in the lives of others!
In this important position, the Case Manager will work in a leadership role for both identified individuals and the overall household!
•assist individuals with their ADLs such as hygiene, cooking meals, etc. and help them to function as independently as possible in a variety of home life situations and community settings
•assisting in the assessment, development and implementation of Individual Service Plan
•ensuring that individuals receive appropriate medical care by coordinating doctor and other health related appointments
•assisting individuals in managing their funds
•completing monthly documentation

Our Ideal Candidate

•Valid driver's license required
•High school degree and 1-2 years experience working with individuals with developmental disabilities required
•Bachelor's degree preferred
