We're seeking a new team member with experience working in a dementia unit, long-term care facility, and/or assisted living community to compliment our community services department.

The CSA will:

•Serve as a friendly visitor to residents who require additional time and attention due to their diminishing cognitive abilities, physical frailty, or mental health status.
•Maintain comprehensive knowledge of identified residents' baselines to assist health services in addressing status changes.
•Act as a liaison between the residents, their families, the on-site assisted living provider, and appropriate New Horizons staff while maintaining strict confidentiality.
•Engage identified residents in appropriate activities.
•Be creative, energetic, upbeat, patient, sensitive and professional throughout activity direction and resident and staff interaction.

Education, experience and other requirements:

•Experience in long-term care or assisted living setting
•Education or experience in working with clients who have dementia
•Ability to manage difficult situations with grace, dignity, and compassion
•CNA/HHA certification preferred, although no direct care will be provided as part of this position
•No felony convictions.
