Reporting to the Options Program Director, the Case Manager Supervisor will develop, manage and implement the Supported Living Program policies and procedures to ensure the program meets and complies with all applicable regulations.  The Case Manager Supervisor will monitor and evaluate the department goals and prepare reports as required to ensure all program record keeping and documentation is accurate and up to date.  Responsibilities will include the supervision, direction, training and development of the program staff consisting of Case Managers, Emergency PCAs, the Program Assistant and Van Driver.  The Case Manager Supervisor will perform assessments of new consumers, assist Case Managers and other program staff with problem situations and provide interim case coordination in the absence of regular Case Managers.<br>
<br>
The successful candidate will have a Bachelor's degree with a concentration in the field of social and/or human services and a minimum of 3 to 5 years supervisory experience.  He/she will have familiarity with community services and the ability to understand and implement Independent Living Philosophy.  Significant experience in personal care attendant service, case management services, and disability service delivery systems is preferred.  Excellent managerial, administrative, orgamnizational and communication skills are required.<br>

Knowledge of homeless shelter and housing first philosophy and knowledge of local, State, and federal housing and human service programs are necessary.<br>

We are seeking mature, responsible, professional, respectful, and supportive individuals when dealing with program participants. These positions require the ability to enforce limitations that have been set for a family/individuals and a willingness to be creative, the ability to maintain confidential information garnered in dealing with guests/program participants and staff, excellent communication and interpersonal skills. Applicants should have a high level of organizational skills with the ability to function independently with minimal supervision.  <br>

The individuals will be responsible for the evaluation and assessment of referrals, the development of case studies of potential participants for housing programs, to help clients get referrals to locate safe, affordable and appropriate housing, to assist clients in applying for subsidies and rental assistance through local housing authorities, regional non-profit agencies, MHFA and HUD, to assist clients who are ineligible for subsidies locate affordable housing in the private market, maintaining current relationships with landlords and private management companies as well as network to form new partnerships with landlords. These positions will also involve advocating  and negotiating  with housing authorities and landlords, assisting shelter based and precariously housed families living in finding affordable housing and rental assistance, maintaining placement and retention statistics, conducting intake interviews and consequence documentation, assisting homeless  families in shelter and hotel with housing search and link up with community based resources, tracking housing participants and submitting subsequent rosters to the billing department and those needing information on program support. Individuals will be attending weekly case management and monthly meetings, making  referrals and maintaining contact with community services, initiating crisis intervention when necessary, coordinating and participating in team meetings, collecting and maintaining data-based statistical information on HMIS computer system, maintaining accurate and in depth case records, ensuring that all relevant information is documented and communicated with other program and management staff, and participating in program monitoring as requested.<br>

•	Ensure timely and high quality completion of all contract deliverables, including work plans, MIS, reporting and community programming<br>
•	Act as spokesperson for coalition in coordination with Brockton’s Mayor’s Office<br>

The Clinical Case Manager will work with an assigned caseload of emotionally disturbed boys and girls between the ages of 4-13. These children are living in one of two group home programs in East Freetown run by the Italian Home for Children. The Clinical Case Manager is responsible for case management of assigned cases and for serving as the primary contact for all family members and collaterals involved with the child. The Clinical Case Manager is also responsible for providing clinical services to children and their families through individual, dyad, group, and family therapy both at the program and in the community. <br>
<br>
Duties Include: <br>
<br>
• Maintain daily contact with outside collaterals and families. <br>
• Relay all significant information to the treatment team. <br>
• Attend conferences, planning meetings, or other relevant clinical meetings as assigned. <br>
• Write comprehensive bio- psycho-social assessments, initial and quarterly treatment plans, progress notes and discharge summaries. <br>
• Conduct intakes into the program including processing all necessary paperwork. <br>
• Provide individual therapy to children, on a weekly basis, or as needed. <br>
• Provide family therapy. <br>
• Lead weekly therapy groups. <br>
• Provide clinical consultation to other treatment team members. <br>
• Assist in providing direct supervision of children when needed. <br>
• Assist in providing physical restraint when needed. <br>
• Serve as one of the members of the intake on–call rotation. <br>
• Accept additional responsibilities as assigned by the Director. <br>

Additional Qualifications: <br>
<br>
• Masters degree in Social Work or a related field. <br>
• LICSW or LMHC required. <br>
• Experience working with a similar population of children and families. <br>
