Build and foster professional relationships with individuals served and members of each individual’s interdisciplinary team to ensure ongoing coordination of services provided by CLASS in accordance with CLASS’ mission. Work in conjunction with other departments, individuals, families, residences, funding sources to ensure that services meet the individual needs and promote individuals choices. The SES Assistant Manager will maintain confidential files for each individual and complete all necessary documentation to meet regulatory agency requirements.<br>

Responsibilities:<br>
<br>
1. Build and foster professional relationships with individuals served and members of the ISP and other teams to maximize an individual’s human dignity, community integration and opportunities for self direction. The Case Manager should routinely spend time with individuals on his/her caseload in all settings across all CLASS services to support them in completing their personal goals for employment. Maintain routine and effective communication with members of the ISP and residence teams regarding progress service planning and incident reporting within the boundaries of confidentiality. Plan for and participate in planning meetings actively advocating for individuals’ needs and contributing to overall goals of the department and individuals supported<br>
<br>
2. Assist the SES Manager in hiring, supervising and training all staff within program. Provide training for Direct Care Staff on competencies, clinical interventions, behavior management and supports as well as being a resource and providing assistance during times of staff or individual issues.<br>
<br>
3. Responsible for determining and administering daily in-house work assignments for both staff and work available, maximizing the Individuals ability to achieve their personal goals. Provide daily oversight of the management of work flow, staffing assignments, behavior management and department priorities as they relate to the overall strategic plan.<br>
<br>
4. Identify through assessments, evaluations and observations individual needs for clinical and employment services, prioritize those needs, and work with the appropriate department to ensure that resources are used efficiently and effectively. Monitor incident reporting to identify needs of support planning and follow-up, as well as all necessary documentation related to all service needs and delivery.<br>
<br>
QUALIFICATIONS:<br>
1. Bachelor’s degree preferred, High School Diploma or GED required.<br>
2. Minimum of 2 years experience in a workshop or employment support position with job development experience preferred. Must be flexible, and actively demonstrate personal desire to enhance the lives of people with disabilities. Must be able to communicate proficiently both orally and in writing to fulfill documentation and service needs of agency.<br>
3. Must pass and maintain required certification programs, to include the following: PAC, First Aid and CPR. Medication Certification as assigned.<br>
4. Transporting of individuals for work/recreational purposes. Includes maintaining a current valid driver’s license, minimum age of 21 for transporting individuals, driver’s clearance per Safe Transport Policy, and regular availability of a vehicle in good working order with current inspection and registration.<br>
5. Flexibility to work with any individual(s) assigned who may require varying levels of support, to include but not limited to, behavioral interventions (may include physical and verbal interventions) and personal care assistance (toileting, eating, etc.).<br>
6. Maintain a flexible work schedule based on the needs of the job.<br>
7. Flexibility to work in all program areas of the agency, accepting internal transfer on a temporary or regular basis based on the needs of the agency and individuals supported as assigned by management personnel.<br>

3.	Maintain confidential records for individuals. Ensure that all necessary documentation is completed in a timely manner, including DHSP, ISP, and RISP progress reports are completed. Route all required documentation to members of the ISP team such as ISP sign off sheets, incident reports, etc. File all required paperwork in the Individual confidential file. *

4.	Actively provide input with team members and coordinate needs with other departments and external referral sources to assure needed and desired services are being provided. *

5.	Participate and assist with intake process and orientation of new individuals to CLASS services. Coordinate or complete assessments/evaluations in order to determine support levels and support planning. Conduct re-evaluations on an as-needed basis. *
<br>

<br>
6.	Lead and participate in the development and implementation of community based employment opportunities that promote personal choice, community inclusion, and dignity and respect while enhancing personal skills development.  Provide transportation as requested by manager.

7.	Assist in providing leadership and supervision of Employment Specialist and Individuals served. 
<br>

<br>
8.	Know and adhere to CARF, QUEST, DMR, DMA, and CLASS policies and procedures.

Reporting to the Assistant Clinical/Program Director, this position is responsible for providing leadership for a transitional and community-based program supporting adults with mental illness on their journey toward recovery and greater independence. The position provides the link to full-time staff regarding resident and Extended Community member concerns, crises, issues, etc. that come up over the weekend period. <br>
<br>
Current weekend shifts at Fellside are approximately two partial weekends per month: one Friday/Saturday shift from 5 p.m. Friday night until 12noon on Saturday and on a separate weekend, 12 noon on Saturday until 3 p.m. on Sunday afternoon. Each shift includes a paid asleep overnight. Substitute weekend and weekday shifts are occasionally available, but vary according to staffing needs. <br>
<br>
Primary responsibilities include, but are not limited to: <br>
•	Ensure that all program participants receive optimal quality of care and support.<br>
•	Be aware of resident schedules (comings and goings) and maintain an accurate account of who is in the building.<br>
•	Respond to emergencies during shift, including use of on-call clinical support.<br>
•	Follow through with any crisis plans put in place by on-call clinical staff. <br>
•	Do medication check-ins as scheduled and assist residents and extended community members in obtaining information for medication concerns.<br>
•	Uphold all policies/guidelines set by Gould Farm and the Boston Area Programs.<br>
•	Complete morning and evening log checklist sheets, including all relevant information during shifts; summarize in weekend transition notes. <br>
•	Transition with staff at the beginning and end of shift.<br>
•	Monitor the general appearance of the house and encourage residents to maintain a clean, orderly, and safe environment. Assist individuals with chores as needed.<br>
•	Assist residents with transportation support by helping them use the mbta.com trip advisor and obtaining directions from websites online. Provide occasional transportation in Fellside vehicle in circumstances authorized by on-call staff. <br>
•	Support Boston Area Program Extended Community members by phone; support any Extended Community members on respite at Fellside during the weekend, which may include implementing particular intervention plans; support an     <br>
                      occasional Gould Farm guest on a trial visit arriving on a weekend.<br>
•	Act as front line person for any maintenance concerns that arise on Fellside property. Contact on-call staff and follow appropriate plan as situation warrants. <br>
<br>
Qualifications: BA degree with 2-3 years in human services desired. Warm and clear communication skills are a must. Good organizational, computer and planning skills and the ability to work independently and collaboratively as a team member. Knowledge of the Boston area is desirable.
