POSITION: Transition Facilitator
LOCATION: Twelve Prescott (Arlington)
DESCRIPTION: (Arlington) Opening for a full-time Transition Facilitator to provide outreach, case management and clinical direct service to young adults age 18-25 who are either living with family or in program-supported apartments. Primary job responsibilities include helping young adults with serious mental illness transition to adulthood via independent living skills training, counseling, psycho education, and teaching emotion regulation and distress tolerance skills. The position includes working some evenings and one weekend day.
QUALIFICATIONS: A bachelor's degree and at least one year of social service experience required. Candidates with graduate-level training or an interest in pursuing a career as a clinician are especially encouraged to apply, as are candidates with training in Motivational Interviewing (MI) or Dialectical Behavior Therapy (DBT).
