Central Massachusetts Independent Physician Association (CMIPA) is a multispecialty physician organization made up of over 200 independent practice physicians in the Worcester, Massachusetts area. We would like to hire a part time Registered Nurse who is a Certified Diabetes Educator.
Job Description:
This position offers an exciting opportunity to become part of our expanding Patient Centered Medical Home (PCMH) Pilot Project. The Certified Diabetes Educator (CDE) will provide individual diabetes self management education (DSME) sessions for our patients with diabetes who are in our PCMH pilot project. As a key member of the interdisciplinary patient care team, the CDE will foster communication and collaboration with the practice staff and CMIPA staff. The CDEˇ¦s practice will be guided by the American Association of Diabetes Educatorˇ¦s Scope and Standards of Practice and Standards of Professional Performance and by the by the goals of our PCMH Pilot Project.
Location: Multi physician office based position with local travel between practices in the Worcester area.
Qualifications:

Current Licensure as a Registered Nurse in Massachusetts
Bachelorˇ¦s Degree in Nursing
Certification as a Diabetes Educator
Three to Five years experience with providing individual DSME in an outpatient clinical setting
Knowledge of Microsoft Applications, specifically MS Outlook and MS Excel

Masterˇ¦s Degree in Nursing or Education
Certified Pump Trainer
